Nickname: Their nickname, if any.
Age: Do I really need to explain?
Date of Birth: Let's take this slowly. ...Birth...day...
Gender: Male or Female?

Ethnicity: Your character's race.
Hair Color: Hair color? Nothing unrealistic, unless they've dyed it.
Eye Color: Again, nothing unrealistic.
Full Description:A full description of your character; at least two paragraphs. That's ten sentences, folks.

Ability: Your character's ability. Don't go crazy with something like "kills with a single stare" or "can control ANYONE'S mind", because I WILL take that as godmodding.
Likes: A simple list of your character's likes.
Dislikes: A simple list of their dislikes.
Characteristics: A full description of their personality. Cover everything. Since all characters should be very indepth and three-dimensional, three paragraphs should be no problem.

Form: What form your daemon takes, if it has settled.
Description: Describe your daemon's looks and personality. If your character's daemon has not settled, this only needs to be one paragraph. If not, it should be two, because you'll be able to describe it physically.
